And every country must be able to stand on its own feet when the next crisis hits. That is the heart of the push toward self-reliance, and it is reshaping how institutions think, spend, and act.

A New Vision for Smarter Global Health Financing

The Global Fund is a prime example. Instead of slow and uneven rollouts, it is pushing new HIV prevention tools to both wealthy and low-income countries simultaneously. This cuts long delays that once cost lives and kept poorer regions behind. It is smart, fast, and focused on impact where it matters most.

Technology is another force speeding up the shift. With better data systems, simple digital tools, and the growing use of AI, institutions can save time and reduce waste. The Global Fund alone plans to reduce costs by an additional 20 percent through these systems. Faster operations mean more saved lives and more resources freed up for the countries that need them most.

The Push From Aid Dependence to Full Self-Reliance

A key aspect of this new era is the transition from long-term aid to full self-reliance. Global health leaders now say openly that countries cannot plan their future around endless donor support. WHO has made this a priority, urging governments to build systems that raise and manage their own resources. One tool is domestic financing through health insurance, which gives countries steady and predictable funding for care.

Another is health taxes on products like tobacco, alcohol, and sugary drinks. Since 2022, at least 116 countries have raised or added these taxes to support stronger health programs.

The Global Fund is also reshaping how it supports countries getting close to transition. Some nations may be on their final three-year grant cycle. Others may need one more round. Instead of cutting support without warning, the Fund now works hand in hand with governments to strengthen public financial systems and unlock new funding streams, including Debt2Health deals and blended finance.

This shift is gaining urgency because donor politics are changing. A good example is the new America First Global Health Strategy. It reframes foreign health assistance as a strategic asset for US interests and expects more co-financing from partner countries.

Africa's Fight for Health Sovereignty

With donor policies shifting, these plans are more important than ever. African leaders are encouraged to negotiate together through the African Union, creating unified expectations for partnerships and reducing the risk of fragmented deals that weaken national interests.

Protecting health sovereignty is a top concern. A major part of this lies in supporting Africa's growing capacity to produce vaccines and medicines. The African Medicines Agency and the Partnership for African Vaccine Manufacturing represent big steps toward independence.
